## The short answer

**Direct care** means you (or an employer on a transparent membership) pay the practice for access and care. Prices are clear up front. The clinician’s incentive is the patient relationship — not a health-plan prior-authorization queue.

Direct care is an umbrella. Under it you will see **direct primary care (DPC)**, **direct specialty care (DSC)**, **cash-pay / episode pricing**, and sometimes **concierge** models that overlap on access but differ on billing.

## What patients usually get

- **Membership or cash fees** published by the practice (monthly, annual, or per visit / episode)
- **Longer visits** and often same-day or next-day access when panels stay intentionally small
- **Transparent prices** for labs, imaging, procedures, or bundled episodes
- A clearer “who do I call?” relationship with a clinician who works for you

You may still keep insurance for hospitals, catastrophic events, or out-of-network specialty care. Many people use direct care **alongside** a high-deductible plan or as their main primary-care home.

## Related models (plain definitions)

- **Direct primary care (DPC)** — membership primary care paid to the practice; insurance usually is not billed for the membership relationship itself
- **Direct specialty care (DSC)** — specialty access with membership, retainer, or cash/episode pricing
- **Cash-pay / episode** — a published price for a visit or procedure without insurance billing for that service
- **Concierge** — marketing umbrella for access-focused practices; confirm what the fee includes and whether insurance is billed
